NDIS personal care shift notes: keeping records audit-ready

Personal care and in-home support generate more individual shift notes than almost any other NDIS category, simply because of the volume of short visits involved. A worker seeing six participants in a day is writing six separate notes, each one needing to stand up if that specific shift is ever questioned.

What actually gets checked

Whether the note's timestamp is close to the actual shift, whether it describes what was actually done rather than a generic template, and whether it matches the hours claimed on the invoice. Notes written well after the shift, or that read identically across different participants, are what draws scrutiny.

The problem with end-of-day notes

Trying to remember the specifics of six separate short visits at the end of a long day almost always produces vaguer, less accurate notes than writing each one up immediately after the visit it describes.
